Create an Account
Visit the official website of the Unique Disability ID. Click on the “Register” button button and provide the following information.
- Personal Detail, including：
➢ your full name in English and regional language.
➢ your parents’ name in English and regional language.
➢ date of birth.
➢ age calculated automatically according to the provided birth date.
➢ gender.
➢ phone number which is used to receive registration confirmation through SMS.
➢ e-mail address which is used to confirm registration.

Wait for Approval and Collect the Card

How to Renew UDID Card
There is a “Valid upto” item on the front of the Unique Disability ID Card.
