What Is PhilHealth ID?

The PhilHealth ID is an identification card issued by the Philippine Health Insurance Corporation for its members. The ID card serves as an important identification that demonstrates your membership. It qualifies you as the cardholder and your qualified dependents for medical services and PhilHealth benefits. You may present the card when dealing with PhilHealth to expedite the processing of claiming any PhilHealth benefits and other transactions.

Types of PhilHealth ID Cards 

There are two types of PhilHealth ID cards you can apply for: the regular PhilHealth ID card and the digitalized PhilHealth Insurance ID card. There are several differences between the two PhilHealth ID cards.

The regular PhilHealth ID card is a paper-based card. This type is issued for free to all PhilHealth members. Since this paper card has no security features, it cannot be considered a valid ID. At the time being, only the regular PhilHealth ID paper card can be obtained from PhilHealth.

The PhilHealth Insurance ID card is a more advanced digitized plastic card that has a QR code and magnetic stripe. It contains more personal information than the traditional regular PhilHealth ID card. This type is optional for employed members. This health insurance ID costs PHP 90 for three-year validity. This card can be considered as a valid ID.

PhilHealth ID Front and Back

The front portion of the PhilHealth ID carries the basic information of the cardholder, including full name, PhilHealth Identification Number (PIN), signature, home address (for the digitized card only), a space for 1 x 1 PhilHealth ID picture, and the validity period of coverage. The back side contains a certification as to the cardholder's identity and entitlement to PhilHealth benefits, along with the signature of the PhilHealth President and CEO.

PhilHealth ID Requirements

To apply for a PhilHealth ID, you must prepare the following documents:

  • Two copies of the PhilHealth Member Registration Form.
  • At least two valid IDs such as a passport, Social Security System ID (SSS ID), or Tax Identification Number (TIN).
  • Original and a photocopy of PSA-issued Birth Certificate.
  • Original and a photocopy of the Marriage Certificate (if applicable).
  • Two copies of 1 × 1 inch ID photos taken in the last six months.

PhilHealth ID Picture Specifications

A PhilHealth ID picture serves to identify the cardholder. The photo must comply with the following official requirements for a successful PhilHealth ID application.

PhilHealth ID Picture Size: 

The size of the photo should be both width and height of 1x1 inch (2.5 x 2.5 cm or 25 x 25 mm). Your head must be between 65 and 68.25% from the bottom of your chin to the top of your head. There should be 1/10 inch from the top of the photo to the top of the hair.

PhilHealth ID Picture Background:

The background should be plain white without any shadows and patterns.
